Doctor Gunter's Clinic is a simple turn-based game that is part visual novel. The game revolves around the titular doctor performing surgeries on his patients. The method of choice for this doctor is, however, punching their kidneys until the kidney stone breaks.

Unlike traditional turn-based combat however, there is a bar similar to the one in Undertale where the player needs to hit in order to deal maximum damage to the kidney stone, and missing deals damage to the patient instead.

At each turn, the Doctor have a certain amount of SP, or Surgical Points, that they can spend. Once they had spend all of their points, or decided that they do not want to perform further action, they can end their turn and the kidney stone will start attacking. There are also various skill that can be performed by using SP, as well as items.

As this is a story based game, the story advances with the levels. There are 5 levels in total, each featuring a different patient suffering from kidney stone. While the difficulty does go up slightly with each level, it is mostly simple enough for casual players.

The five patients in the story are the Black Market Dealer, the Old Lady, the Nomad, the Bodybuilder and Doctor Gunter himself. That's right, the goal of the mysterious Doctor is to use his patients to further his skills so he can cure his own kidney stones!
